Dear Valued Patients,

This is not an easy letter to write, and it was not an easy decision to make, but the time has come to cross this bridge. Nothing in my professional life has mattered more to me than my patients, and I have always considered every one of you part of my extended family. When a patient sits across from me and entrusts me with something as important as his or her health, I have tried, in every recommendation and every decision, to treat that trust the way I would want a member of my own family treated — with the same care, the same honesty, and the same commitment to getting it right.

After 33 years caring for patients from this same Fairfax location, I have made the difficult decision to close my solo subspecialty practice, Al-Juburi Urology, PLLC. This letter serves as formal notice of that closure and explains what it means for your ongoing care and your medical records.

October 30, 2026, will be my last day seeing patients in the office. From October 31 through the end of December 2026, the office will remain open by appointment only, to assist with administrative matters, billing questions, and medical records requests. After that time, the practice will close.

Where I'm Going

Beginning November 1, 2026, I will be joining UVA Health on a part-time basis, which will allow me to continue doing what I love most — caring for my patients and my community. I will primarily be practicing out of the Manassas location:

Your Medical Records

Whether or not you choose to continue your care with me, you have the right to have your medical records sent to a health care provider of your choice, or to receive a copy of your records directly. We will charge the actual cost of copying and mailing or delivering the records; based on that, we estimate a fee of $25.00 for records requested directly by a patient. There is no charge to transfer your records to another provider of your choosing.

To request your records, please email us at aljuburiurology100@gmail.com, call our office at (703) 691-4666, or send your request by fax to (703) 691-2459. Requests are checked regularly and will be fulfilled within 30 days. Please note that standard email is not an encrypted or fully secure method of transmission; if you would prefer an alternate method for sending your records, please let us know when you submit your request.

After the practice closes, I will remain personally responsible for your records, which will be kept in secure storage. Records will be retained for a minimum of six years from your last date of treatment. For patients who were minors, records will be retained until the patient turns 18, or for six years from the last date of treatment, whichever is longer.

Prescriptions and Ongoing Care

If you have an active prescription that will need refilling or are in the middle of an ongoing treatment plan, please contact my office as soon as possible so we can help arrange for continuity of care.
